With Athletes Information Release forms being valid for three years, it is essential you provide us with any changes in medical conditions or anything else that is vital to the health of the athlete. If you change your home or work location, please notify us immediately. In some cases, there may be a delay in changing take home routes and missing a practice will be necessary. If possible, please give us two weeks lead-time on any changes.
